Aim: Total thyroidectomy is one of the surgical procedures which is located in the upper row in surgery, performed in general surgery and can cause serious complications which are very low mortality but are specific to itself and may impair the quality of life of the patients. The most common complications are hypocalcemia, vocal cord paralysis, bleeding, infection and adjacent organ injuries. In this study, we aimed to determine retrospectively the results of thyroid volume effects in patients who underwent total thyroidectomy and to identify the causes leading to postoperative complications and to determine the precautions for these reasons.Material And Methods: In our study, 263 patients at adult aged who had resorted to General Surgery Clinic with surgical indications due to benign or malign diseases of the thyroid gland, at Erciyes University Medical Faculty Hospital between January 2011 and October 2015. All the patients' postoperative thyroid gland weight was measured by Pathology Department. The hospital automation system and patients's files were reviewed retrospectively and data were recorded. Postoperative complications such as demographic characteristics, preoperative USG, scintigraphy and imaging methods such as tomography, preoperative laboratory values, FNABs, preoperative diagnosis, postoperative pathologic diagnoses and weight of the specimens, hemorrhage, seroma accumulation, hypocalcemia, vocal cord paralysis and infection were investigated. Results: Of the 263 patients, %70,7(n: 186) were female, %29,3(n: 77) were male and the mean age was 49.8 y (18y - 92y). The mean thyroid gland weight was 72.2 gr (10 gr -500 gr). The incidence of total complications was 24.3% and these were determined as temporary and permanent hypocalcemia, vocal cord paralysis, hemorrhage and seroma. The rate of transient hypocalcemia was 20.1% (n = 53) and the rate of permanent hypocalcemia was 1.5% (n = 4). The transient VCP rate was 0.3% (n = 1) and no permanent VCP was seen in the patients. In patients without postoperative hypocalcemia, median thyroid weight was measured as 50 gr, in hypocalcemia group as 40 gr (p = 0,283). There was no significant relationship between variability in thyroid weight and postoperative hypocalcemia. However, cervical lymph node dissection (p = 0.006) and servical dissection site (p = 0.031) was statistically significant in terms of postoperative complication development. Female sex and LND were found to be risk factors for the development of postoperative complications. Conclusions: The most common complications after total thyroidectomy were hypocalcemia (21.6%), hemorrhage (1.5%), seroma (0.7%) and vocal cord paralysis (0.3%). Cervical LND with total thyroidectomy and female sex were statistically significant risk factors for postoperative complications. There were no statistically significant results in our study for the risk factors that increase incidence of postoperative complications incompatible with literature findings, such as thyroid gland weight, presence of thyroid surgical story, malign diseases of thyroid gland, cervicomediastinal placement, hyperthyroidism.
