2.SUMMARYThe Effect of Dıfferent Body Positions and Speaking On Blood Pressure In Patıents wıth HypertensıonThis descriptive and analytic study was conducted to determine the effect of different body positions and speaking on blood pressure (BP) in patients with hypertension. The population of the research consisted of hypertensive patients who were treated as outpatients and inpatients between February and June 2011 at the Cardiology clinic of a university hospital and the study sample consisted of 162 hypertensive patients who met the criteria of inclusion and agreed to participate in the research. The data were collected with a patient identification form and BP Measurements. BP measurement was done with aneroid sphygmomanometer. Measurements were done on the left and right arms, respectively, the patient sitting, talking, lying, lying legs crossed at the level of the ankle and measurement arm being supported by a small pillow at the level of the mid-line of the sternum. The patients were allowed to rest for 3 minutes (min) at the end of each measurement, BP measurement took 30 minutes. Analysing the data, Chi-square test was used for percentage, frequency, mean, Student's t-test, analysis of variance and categorical values. On the measurents conducted on the right arm while the patient was speaking, it was found that DBP was high (77.1±13.3) (p=0.003). It was seen that the measurements made by crossing legs while lying at the level of the ankle didn?t change the BP results. It was observed that while sitting, DBP was high (76±12.3) (p=0.021) on the left arm BP measurement, and while lying, it was low on the right (72.8±11.6) and left (72.7±11.1) arm measurements (p=0.003, p=0.021). As a result, it was confirmed that the positions taken into consideration and BP values measured while speaking didn?t make any difference on SBP; however, DBP values were high on the left arm measurement in sitting position. Araştırma hipertansiyonlu hastalarda farklı vücut pozisyonları ve konuşmanın kan basıncı (KB) üzerine etkisini belirlemek amacıyla tanımlayıcı ve analitik olarak gerçekleştirildi. Araştırmanın evrenini bir üniversite hastanesinin Kardiyoloji kliniğinde şubat ile haziran 2011 tarihleri arasında ayaktan ve yatarak tedavi edilen hipertansiyonlu hastalar, örneklemini ise, örnekleme alınma ölçütlerine uyan ve araştırmaya katılmayı kabul eden 162 hipertansiyonlu hasta oluşturdu. Veriler Hasta Tanıtım Formu ve Kan Basıncı Ölçüm Değerleriyle toplandı. KB ölçümü aneroid sphygmomanometer ile yapıldı. Ölçümler sırasıyla; hasta otururken, konuşurken, yatarken, yatarken ayaklar ayak bileği hizasında çaprazlanarak ve ölçüm yapılan kol küçük bir yastıkla sternumun orta hattı hizasında desteklenerek, sağ ve sol koldan yapıldı. Her ölçüm sonunda hastaların üç dakika (dk) dinlenmeleri sağlandı, KB ölçümleri 30 dk sürdü. Veri analizinde yüzdelik, frekans, ortalama, student t testi, varyans analizi ve kategorik değerler için ki-kare testi kullanıldı. Konuşurken sağ koldan yapılan ölçümlerde DKB'nin yüksek (77.1±13.3) (p=0.003) olduğu bulundu. Yatarken ayakları ayak bileği hizasında çaprazlayarak yapılan ölçümlerin KB sonuçlarını değiştirmediği görüldü. Otururken sol kol KB ölçümünde DKB'nin yüksek (76±12.3) (p=0.021) olduğu ve yatarken, sağ (72.8±11.6) ve sol (72.7±11.1) koldan yapılan ölçümlerde DKB'nin düşük, olduğu tespit edildi (p=0.003, p=0.021). Sonuç olarak; baz alınan pozisyonlar ve konuşurken ölçülen KB değerlerinin SKB'de fark yaratmadığı ancak oturur pozisyonda sol koldan ölçümde DKB değerlerinin yüksek olduğu saptandı.
