Hypoxic ischemic encephalopathy is very important as it causes permanent central nervous system damage and neonatal mortality or static encephalopathy and mental retardation in future stages. Since S100B and Tau proteins increase during cerebrovascular events such as head injury, coma and perinatal asphyxia, cerebrospinal fluid and serum levels of these proteins have been used to determine diagnosis and prognosis.In this study the aim was to determine the importance of serum S100B and tau proteins in the diagnosis and prognosis of hypoxic ischemic encephalopathy in term newborn infants. The study group consisted of 20 term newborn infants with a diagnosis of hypoxic ischemic encephalopathy and 20 term newborn infants with normal physical examination findings were included as the control group. For S100B and tau proteins, serum samples were taken twice in the first two hours and on the 48th hour postnatally from the subjects with hypoxic ischemic encephalopathy. Blood samples of the control group were taken once on the 2nd hour postnatally. S100B protein levels of the study group were not significantly high compared to the control group. Although Tau protein levels were higher compared with the control group, the difference was not statistically significant. Tau protein levels obtained at 2nd hour postnatally from infants with stage II (p=0,017) and at 48th hours of life from infants with stage II, were significantly higher than stage III (p=0,035) infants and those in the control group (p=0,001). While sensitivity was high (95%), specificity (50%) was found low in S100B protein obtained at the 2nd hour. Sensitivity (75%) and specificity (%55) at the 48th hour were low. In Tau protein obtained at the 2nd hour, sensitivity (75%) and specificity (%65) were low, while in tau protein obtained at the 48th hour specificity (85%) was found high and sensitivity (55%) was low. Neurological examination performed at one month of age revealed that pathological findings were more frequently encountered in those infants with stage II and III hypoxic ischemic encephalopathy compared to those with stage I. In those infants whose neurologic examination were pathologic, protein S100B and Tau levels were higher. As a result, it was seen that especially Tau protein is a relatively significant indicator in determining the neurologic dysfunction in infants with hypoxic ischemic encephalopaty. We thought that the reason why we found S100B protein meaningless, is due to the lack of case number in our study. Hipoksik iskemik ensefalopati, merkezi sinir sistemi hücrelerinde kalıcı hasara, neonatal mortaliteye ya da ileriki dönemlerde statik ensefalopati ve mental geriliğe yol açması nedeni ile çok önemlidir. S100B ve Tau proteinleri beyin hasarını ilgilendiren serebrovasküler olaylar, kafa travması, koma ve perinatal asfiksi gibi durumlarda artmakta olup, beyin omurilik sıvısı ve serum düzeyleri tanı ve prognoz belirlemede kullanılmaktadır. Bu çalışmada, Hipoksik iskemik ensefalopatili term yenidoğanlarda nörolojik disfonksiyonun göstergesi olarak S100B ve Tau proteinlerinin yeri araştırılmıştır. Çalışma grubunu hipoksik iskemik ensefalopati tanısı alan 20 term yenidoğan ile kontrol grubu olarak fizik muayenelerinde patolojik bulgu saptanmayan sağlıklı 20 term yenidoğan oluşturmaktaydı. Olgulardan S100B ve Tau proteinleri için ilk 2 saat içinde ve 48. saatte olmak üzere iki kez serum örneği alındı. Kontrol grubundan ise doğum sonrası 2. saatte tek kan örneği alındı. Çalışma grubunda 2. ve 48. saatte saptanan protein S100B düzeylerinde kontrol grubuna göre anlamlı bir yükseklik yoktu. Tau protein düzeyleri kontrol grubuna göre yüksek olmasına rağmen istatistiksel olarak anlamlı farklılık saptanmadı. Tau proteininde, 2. saatte Evre II'de kontrol grubuna göre (p=0,017) 48. saatte Evre II'de, Evre III'e (p=0,035) ve kontrol grubuna (p=0,001) göre istatistiksel olarak anlamlı bir yükseklik saptandı. S100B proteininde ikinci saatte alınan örneklerde duyarlılık yüksek (% 95) bulunurken özgüllüğü düşük (% 50) saptandı. Kırksekizinci saatteki duyarlılık (% 75) ve özgüllük (% 55) düşüktü. Tau proteininde ise ikinci saatte duyarlılık (% 75) ve özgüllük (% 65) düşük saptanırken 48. saatte özgüllük yüksek (% 85) duyarlılık düşük (% 55) saptandı. Postnatal 1. ayda yapılan nörolojik muayenede Evre II ve Evre III'te Evre I'e göre patoloji yönünden istatistiksel olarak anlamlı bir yükseklilik saptandı. Nörolojik muayenesi patolojik olan olguların serum protein S100B ve Tau protein düzeyleri normal olan gruba göre daha yüksekti. Sonuç olarak özellikle Tau proteininin hipoksik iskemik ensefalopatili olgularda nörolojik disfonksiyonun göstergesi olarak bir belirteç şeklinde kullanılabileceğini düşünmekteyiz.
