Caustic esophagus burns are still a clinical challenge in these days. Especially treatment modalities against development of stenosis after esophagus burns are still important. The aim of this study is to search the effect of ibuprofen treatment, a non-stereoid antiinflammatory drug, on esophagus burns and transform the results to clinical treatment. In this study 30 Wistar albino type rats were used. Rats were randomly divided into 3 groups as sham, burn and ibuprofen groups. In burn and ibuprofen groups; an esophagus burn was created by 10% NaOH solution in a abdominal segment of about 3 cm according to standart burn model which is described by Gehanno and Guedon. To rats in the sham group, isotonic solution was given. Ibuprofen was given perorally at 90 mg/kg/day dosage to the ibuprofen group. Twenty-eight days later an esaphagogram was taken. The rats were sacrified the same day. The burn created segment was extracted and divided into two equal parts for biochemical and histopathologic examination. In the tissue samples; biochemically hydoxypirolin (HP), malondialdehide (MDA) and histopathologically connective tissue score (CTS) and stenosis indexes (SI) were evaluated. At the end of the study, there was weight gain in sham and ibuprofen groups and weight loss in burn group. In the esaphagogram no stenosis was seen in esophagus in sham group, and stenosis in distal esophagus was observed in burn group. In the ibuprofen group a slight narrowness which didn?t affect the passage was observed. Hydoxypirolin levels assessed from esophagus wall in the ibuprofen group (3,28 ±0,27 µg/mg wet tissue) was higher than the sham group (1,54± 0,08 µg/mg wet tissue) but it was significantly lower than the burn group (4,82 ±0,60 µg/mg wet tissue). Malondialdehyde levels were; 14.78 ± 1.43, 28.75 ± 3.81, 23.97 ± 4,80 nmol/g wet tissue in the sham, burn and ibuprofen groups, respectively. Although MDA levels were significantly increased in burn group against sham group, in the ibuprofen group no important difference was seen between sham and burn group. In histopathologic examination the connective tissue score in sham, burn and ibufen groups were 0.00 ± 0.00, 2.62 ± 0.37, 1.50 ± 0.26, respectively. In connective tissue score there was a significant increase in burn group compared with the sham group (p<0,01). Although an increase in ibuprofen group compared with sham group was determined, there was a significant decrease in connective tissue score in ibuprofen group compared with the burn group. (respectively p<0.01 and p<0.01). Stenosis index (SI) was found as 0.37 ± 0.02 in sham group, 0.84 ± 0.02 in burn group and 0.67 ± 0.03 in ibuprofen group. In the burn group the SI was significantly high while it was significantly decreased in the ibuprofen treated group compared with burn group. In conclusion; with the usage of ibuprofen treatment for experimental esophagus burn model, there were statistically significant decrease in HP, CTS and SI compared with burn group and as a clinical reflection of this the rats in this group there was weight gain and in their esophagogram there was no stenosis blocking the lumen. According to the data obtained from this study it was accessed that; even treatment with ibufen doesn?t provide full improvement it gives significant amelioration compared with untreated group, so it can be used simply to prevent/decrease the stenosis in acute-subacute esophagus burns? treatment and to decrease fibrosis after dilatation on the patients taking dilatation therapy and it can be used to provide healing in short term.
