In this study, we aim to compare implantation arthroplasty technique with resection interposition technique with regard to clinical and functional results for high grade hallux rigidus patients according to Coughlin Shurnass classification by using their preoperative and postoperative AOFA scores and range of motion of the first metatarsophalangeal joint.Material and Method: In this study, 19 feet of 19 patients were included. AOFA scores and range of motion of 1st metatarsophalangeal joint of all patients were recorded. Implantation arthroplasty was applied to 11 patients to whom active and passive joint motion had been started immediately at the postoperative period. On the other hand, to 8 patients interposition arthroplasty was applied to whom joint motion rehabilitation was started after 3 weeks of nonweightbearing period. All results of two groups were analysed statistically using Mann-Whitney U test and Wilcoxon test. Resuts: Although postopertive joint extension and AOFA scores were statistically better in both groups after surgery, there was not any difference between groups in that manner. There was also no any statistically difference in both groups wtih regard to flexion of the 1st metatarsophalangeal joint. Results: Because of similar functional results and similar complication rates, resection implantation arthroplasty surgery seems as a first choice after failed conservative treatment when cost-effectivity and severity of complications are considered. However, to demonstrate the clinical and functional difference between the two methods, larger groups of patients with longer follow-up are needed to be evaluated in further clinical studies.
