PURPOSE: The purpose of this study is to investigate the effect of high velocity low amplitude (HVLA) upper cervical spinal manipulation on neck range of motion, neck muscle strength, pain intensity, and balance.METHODS: The healthy individuals included in the study were randomly divided into two groups. Both groups were evaluated with Microfet-2, CROM and Biodex Balance System (BBS) before and after the intervention. A total of 40 people were included in the study. In the experimental group the upper cervical HVLA manipulation was applied consisting of 20 people, sham manipulation was applied to the control group consisting of 20 people. As a result of single intervention performed in both groups, the immediate effect on balance, cervical range of motion and cervical muscle strength were evaluated with the same methods and the results were compared.RESULTS: Initially, complementary data and cervical joint movements (CROM), cervical muscle strength (Microfet-2), Visual Analogue Scale (VAS), General Stability Index (OSI) and Anterior Posterior Stability Index (APSI) for balance assesment were homogeneous in terms of both groups measurements (p>0.05). There was a only statistically significant difference for eyes closed Medio-Lateral Stability Index (MLSI) value when two groups were compared (p<0.05). After intervention, there was no statistically significant difference between the groups for the VAS values, Microfet-2 values and the other balance evaluations (p> 0.05), there was significant difference for the CROM in terms of rigth and left lateral flexion values (p<0.05). MLSI value was decreased after the intervention. In the MSM group, a statistically significant difference was observed in the increase of right and left lateral flexion in the CROM measurement, while a significant difference was observed in the increase in right and left rotation angles in the sham group (p<0.05). There was no statistically significant difference between the two groups before and after the measurement of Microfet-2 in MSM and sham group (p>0.05). In the MSM group, a statistically significant difference was found when evaluated the double feet balance eyes open OSI and APSI; eyes closed OSI, APSI, MLSI; in dominant foot eyes closed OSI, APSI and MLSI values observed in the decrease (p<0.05). There was a statistically significant difference in the decrease in VAS (p<0.05). There was no statistically significant difference between the open eyes of the dominant foot and open and closed eyes of non-dominant foot (p>0.05). While there was a significant difference in the decrease in APSI values in the sham group, there was no statistically significant difference in the other balance measurements (p>0.05). There was no significant difference in VAS value (p>0.05).DISCUSSION: In our study, it was observed that manual spinal manipulation applied to the upper cervical region in healthy individuals increase double feet eyes open and eyes closed balance and dominant foot eyes closed balance. It has been found that MSM reduces pain and increases lateral flexion in the range of motion of the neck. In future studies, it is recommended that patients with no risk factors with loss of balance should be done with more cases, comparison of long-term and short-term effects of spinal manipulation, and comparison of measurements with different sensitive device. Keywords: Chiropractic, Manipulation, Upper Cervical, Balance.
