Histopathological evaluation of sprinolacton and surfactant treatment in rabbits with acute pulmonary injury caused by acid aspiration and the effects of the treatment on HMGB1, CRP, lL1-ß ve TNF-? levels.ARDS (acute respiratory distress syndrome) is the dysfunction of the pulmonary capillary endothelium and alveolar epithelium.In this study we have investigated the efficiency of intragasrtic sprinolacton and intrathecal surfactant treatment evulating and comparing the blood levels of HMGB1 (high-mobility group box-1), IL1ß (interlökin-1 beta), TNF? (tumor necrosis factor alfa), and CRP (C-reactive protein) which are acute phase reactants in experimental acute pulmonary injury model.This study included 40 New zeland rabbits which were randomly divided into five groups (Group C control group, Group T tracheostomy group, Group A: ARDS group, Group S1 sprinolacton group, group S2 surfactant group). Subjects have been fasted for 8 hour befıre the experiment and have been anesthetised with 50mg/kg sodium penthal.Only blood samples are taken from group K.Blood samples are taken from group T after exposure of the femoral arteries at 6th and 24 th hours.Tracheostomy is performed on group a and 0.4 mL/kg 0.1 mol/L of HCl(Hydrochloric acid) (pH ~ 1.25) is given intratheccally through the endotracheal cannule in order to cause pulmonary injury and volüme controlled mechanic ventilation is performed.Tracheostomy is performed on group S1 and spironolacton is given through nasogastric tube (100 mg/kg) 30 minutes after the pulmonary injury.Tracheostomy is performed on group S2 and 100mg/kg surfactant is given intratracheally 30 minutes after the pulmonary injury.Tidal volume of the mechanical ventilation was 8 mL/kg and respiratory frequency was 40/min. At the end of the experiment all of the subjects were sacrified by intraperitoneal injection of sodium penthotal and lungs are excised for pathological evulation.Difference between the groups were anaylsed using Kruskal-Wallis test. Comparison of the foups were done by using the Mann-Whitney U test.Group A had statistically significantly lower PO2/FİO2, pO2, SpO2, pH values (p<0,005). Group S1 and S2 had statistifically significantly higher values(p<0,001). Group A had statistifically significantly higher PCO2 values. (p<0,005). Group 1 and Group 2 had statistically lower pCO2 values (p<0,05). Group A had statistifically significantly higher HMGB1, CRP, IL1ß and TNF? values (p<0,001). Group 1 and Group 2 had statistically significantly lower values. (p<0,05).Group T had better pulmonary injury scores then group A, group S1 and group S2 had lower score but the difference was not statistically significant (p>0,05).In conclusion this study showed that intratracheal surfactant and intragastric sprinolacton improved the arterial blood gas values and decreased the blood levels of acute phase reactants which increase in the presence of inflammation and provided histopathologic recovery of the lung in experimental lung injury model.Key Words: Acid aspiration, ALI, ARDS, Spironolactone, Surfactan, HMGB1, CRP, IL1ß ve TNF?.

Tavşanlarda asit aspirasyonu ile oluşturulan akut akciğer hasarında Spironolakton ve Sürfaktan tedavisinin histopatolojik incelenmesi ve HMGB1, CRP, lL1-ß ve TNF-? üzerine etkisi:ARDS (Akut sıkıntılı solunum sendromu) akciğerde direk veya indirek etkiyle gelişen pulmoner kapiller endotelin ve alveol epitelinin fonksiyon bozukluğudur.Çalışmamızda deneysel olarak geliştirilen akut akciğer hasarında intragastrik spironolakton ve intratekal surfaktan tedavisi ile akut faz reaktanı olan HMGB1 (high-mobility group box-1), IL1ß (interlökin-1 beta), TNF? (tümör nekrozis faktör alfa), CRP (C-reaktif protein) plazma düzeylerini karşılaştırıp tedavide kullanım etkinliğinin olup olmadığını ve bu tedavinin akciğerde oluşan histopatolojik değişiklikler üzerine olan etkisini inceledik.Çalışmamızda Yeni Zellanda türü 40 tavşan kullandık.Denekler rastgele 5 gruba (Grup K: Kontrol grubu(n=8), Grup T: Trakeostomi grubu(n=8), Grup A: ARDS grubu(n=8), Grup S1: Spironolakton grubu(n=8), Grup S2: Surfaktan grubu(n=8) ayrıldı. Denekler çalışmadan önce 8 saat aç bırakılıp intraperitoneal olarak 0.06 mgr atropin sulfat ve 50 mg/kg pental sodyum ile anestetize edildi.Grup K'daki deneklerden sadece kan örnekleri alındı.Grup T'den femoral arterleri açıldıktan sonra 6 ve 24.saatlerde kan örnekleri alındı.Grup A'ya trakeostomi açıldıktan sonra akciğer hasarı yapmak için 0.4 mL/kg 0.1 mol/L HCl(Hidroklorik asit) (pH ~ 1.25) intratrakeal yolla damla damla endotrakeal kanülden verildi ve volüm kontrollü mekanik ventilasyon uygulandı.Grup S1'de trakeostomi açılıp akciğer hasarı yapıldıktan 30 dakika sonra tavşanlara nasogastrik sonda takılarak spironolakton (100mg/kg) verildi.Grup S2'de trakeostomi açılıp akciğer hasarı yapıldıktan 30 dakika sonra 100mg/kg surfaktan trakea içine uygulandı.Deneklere mekanik ventilasyon tidal volumü (Vt) 8 mL/kg ve solunum frekansı (f) 40/dak olarak uygulandı. İşlem sonunda denekler intraperitoneal pental sodyum kullanılarak sakrifiye edildi ve histopatolojik olarak incelenmek üzere akciğerleri çıkartılıp patolojiye gönderildi.Gruplar arası farklılık non-parametrik bir test olan Kruskal-Wallis testi ile analiz edildi. İkili grup karşılaştırmaları için ise Mann-Whitney U testi kullanıldı. Verilerde istatistik anlamlılık düzeyi p<0,05 olarak alındı.PO2/FİO2, pO2, SpO2, pH, değerleri Grup A'da diğer gruplara göre istatistiksel olarak anlamlı düşüktü (p<0,001). Grup S1 ve S2 `de ise diğer gruplara göre istatistiksel olarak anlamlı yüksek idi (p<0,001). pCO2 değerleri Grup A'da diğer gruplara göre istatistiksel olarak anlamlı yüksek idi (p<0,001). Grup S1 ve S2 `de diğer gruplara göre istatistiksel olarak anlamlı düşüktü (p<0,05). HMGB1, CRP, IL1ß ve TNF? değerleri grup A'da diğer gruplara göre istatistiksel olarak anlamlı yüksek bulundu (p<0,001). Grup S1 ve grup S2'de diğer gruplara göre istatistiksel olarak anlamlı düşüş görüldü (p<0,05).Akciğer hasarı skorlaması sonucunda grup T ve grup A'da skor yüksekti. Grup S1 ve grup S2'de skor düşüktü, fakat gruplar arasındaki istatistiksel olarak anlamlı fark bulunmadı (p>0,05).Sonuç olarak, çalışmamızda akut akciğer hasarı oluşturulan deneklerde intratrakeal surfaktanın ve intragastrik spironolaktonun arter kan gazı değerlerini iyileştirdiği, inflamasyon sonucu yükselen akut faz reaktanlarını düşürdüğü ve akciğerde histopatolojik iyileşme sağladığı gözlendi.Anahtar Kelimeler: Asit aspirasyonu, ALI, ARDS, Spironolakton, Surfaktan, HMGB1, CRP, IL1ß ve TNF?.
