Soft Tissue Changes During Distraction Osteogenesis. stanbulUniversity, Institute of Health Science, Department of Oral and Maxillofacial Surgery.Ph.D Thesis. stanbul. 2006.Distraction osteogenesis is a valuable method in the reconstruction of bony defectswhich gains more importance every day in today' scientific world. In this study, it wassaimed to determine the soft tissue changes in different consolidation periods ofdistraction osteogenesis which was gained by different amounts of distraction. As aresult of this, fifteen rabbits in three groups were evaluated in five differentparameters. After distraction application on the right side of the rabbits' mandible, differences in the number of masseter muscle' fibers and nuclei (according toslocalization and total number) were investigated. An external device, which wasdesigned by us, was placed on the right sides of the mandibles' the rabbits. On all ofofthe study groups, post-operative latent period was determined to be four days.Distraction was applied twice a day with 0.25mm in each turn, thus thetotal distraction length was 0.5mm per day. The first group was distracted for six days(3mm), the second group was distracted for eleven days (5.5mm), and the third groupwas distracted for sixteen days (8mm). Additionally, the rabbits'masseter musclelengths were initially determined to be 27.5mm approximately and afterwards in eachanimal, the retrieved percentage difference in length was calculated (in the first group:11%, in the second group: 20%, and in the third group: 29%). In all of thestudy groups, consolidation periods were determined by multiplication of the distractionperiod by two. All of the rabbits were sacrified at the end of the consolidation periods.Specimens were retrieved from the distracted right sides (study) of the mandibles andthe left (control) sides of the mandibles. In histological analyses, tissue specimens weredyed with hematoxilen-eosin and underwent a series of chemical procedures after whichthey were examined with an Olympus CH20 light microscope. In thehistomorphometric analyses, it was concluded that the number of muscle fibers andnuclei on the study side increase proportionally with the distraction period. The localization of the nuclei migrated from the perifere to the centrum. The changes inthese parameters were interpreted to be the newly formed muscle fibers, characterizedwith hyperplasia and hypertrophia. It was determined that the regeneration in the firstgroup continued in it' natural pattern; in the second group, there seemed to be asdegeneration rather than a regeneration; and in the third group there seemed to be abalance between regeneration and degeneration processes. According to all of theseanalyses, the best muscle tissue adaptation during distraction process was observed inthe first group where eleven percent lengthening was achieved. With the help of theresults of this study, it may be concluded that when a massive amount of bone is desiredto be obtained, instead of distracting a bone at once, it may be better to divide the totalamount of distraction into several time periods.Key words: Distraction osteogenesis, distraction histogenesis, rabbit, muscle, adaptation.

xivÖZETYazdurdıyev B. Distraksiyon osteogenezisinde yumu ak doku de i iklikleri. stanbulÜniversitesi Sa lık Bilimleri Enstitüsü, A ız Di -Çene Hastalıkları ve Cerrahisi ABD.Doktora Tezi. stanbul. Bu amaçla 15 adet tav an, üç grup halinde,be farklı parametrede de erlendirildi. Uzatma i lemi günde 0.5mm (2 kez 0.25mm) olmak üzeregerçekle tirildi. Çekirdeklerin lokalizasyonundaise, periferden merkeze göç etti i izlendi. Anahtar Kelimeler: Distraksiyon Osteogenezisi, Distraksiyon Histogenezisi, Tav an,Kas, Adaptasyon.
