BOLUM VI ÖZET Araştırma, Menopoza Özgü Yaşam Kalitesi Anket Formu' nun (MENQOL) Türkiye'deki geçerlik ve güvenirliğini test etmek ve klimakterium dönemindeki kadınların yaşam kalitelerini, umutsuzluk düzeylerini ve etkileyen faktörleri belirlemek amacıyla tanımlayıcı ve kesitsel olarak planlanmıştır. Araştırmanın örneklemini; Dokuz Eylül Üniversitesi Tıp Fakültesi Hastanesi Kadın Hastalıkları ve Doğum A.D. Kadınların %90.0'ı evli, %35.0'i yüksekokul/fakülte mezunudur. Çalışmayan %85.0 kadının %32.5'i emeklidir.

176 SUMMARY THE INVESTIGATION OF HOPELESSNESS LEVELS AND QUALITY OF LIFE IN CLIMACTERIUM OF THE WOMEN This descriptive and sectional study was planned to test the validity and reliability of the Menopause-Specific Quality Of Life Questionnaire (MENQOL) in Turkey and to determine the quality of life, hopelessness levels and effective factors of climacteric women. The study sample consisted of 120 women with natural menopause, who admitted to the Department of Obstetrics and Gynecology Menopause Policlinic, Dokuz Eylül University, Faculty of Medicine between 1 June-30 Agust 2003, and whose last menstrual bleeding had been at least one year ago and who had not been using HRT for the last six months before their participation in the study. As data collection tools, an interview form concerning the demographic and climacterium characteristics of women, a Menopause-Specific Quality Of Life Questionnaire (MENQOL), and Beck Hopelessness Scale were used. In the assessment of the data ANOVA, Student t-test (İndipendent t test), Chi- Square test, and Correlation test were used for the statistical analyses. Five specialists were consulted for the language validity and 10 specialists were consulted for the content validity of MENQOL. Kendall Coefficient of Concordance was found to be (W: 0.393, p< 0.001). Test-retest reliability coefficient was found for vasomotor domain to be 0.93, for psychosocial domain to be 0.97, for physical domain to be 0.97 and for sexual domain to be 0.88. Of MENQOL, the correlations between the two halves for the first and second application were 0.57 and 0.58 respectively; Guttman Split-half reliability coefficients were 0.68, 0.68 respectively; Spearman Brown reliability coefficients were 0.72, 0.73 respectively;177 and 15 items in Part 1 Cronbach Alpha Value was 0.91, 0.91 and 14 items in Part 2 Cronbach Alpha Value was 0.70, 0.71. Of women, who participated in the study, 67.5% was in the age range of 50- 59 years whereas their mean age was 53.00±4.92 years and mean menopause age was 47.90+3.99 years. Majority of the women were married (90.0%) and 35.0% of them were the graduates of high school/faculty. Of the unemployed 85.0% of women, 32.5% were on retirement. Of women with a systemic disease (57.5%), 43.2% were with hypertension. Of women, 100% were not consuming alcohol whereas 76.6% were non-smokers. The mean age of the women, who smoked cigarettes, was 46.46±3.30 years and that of the non-smokers was 48.33±4.10 years. A significant relationship was found between the menopause age and women's cigarette smoking (x2: 5.673, p<0.05). The women in our study, who smoke, had undergone a natural menopause two years earlier than that of non-smokers. Of women, 54.2% were informed about climacterium whereas of this 54.2%, 38.5% were informed by the doctor while only 4.6% were informed by nurse/midwife. There was a significant difference between the mean score of quality of life and age groups, marital status, educational levels, and presence of a systemic disease of the climacteric women. Similarly, a significant difference existed between the mean score of hopelessness and their educational levels and occupations. The effect of the women's perception of menopause on the mean score of quality of life and hopelessness has been found statistically significant. It has been determined that 44.2% of the women perceive the menopause as `distress-stress` and that their scores of quality of life were low and those of hopelessness were high.178 It was detected that there was a positive relationship among the domains regarding the quality of life of the women in this study. Seriesly (r=0.297, r=0.359, r=0.256, r=0.641, r=0.278, r=0.378, p<0.05). A positive relationship between the mean score of hopelessness and women's psychosocial, physical, and sexual domains has been detected. Seriesly (r=0.490, r=0.356, r=0.212, p<0.01). No significant difference between vasomotor domain and the mean score of hopelessness was detected. It has been found that the higher the mean scores obtained from psychosocial, physical and sexual domains were, the higher the mean score of hopelessness. That is, a high mean score of quality of life indicated a low quality of life of women. In the light of the findings obtained from the women, who participated in the investigation, in informing of women about climacterium, employing nurses in menopause policlinic and providing nurses to maintain their tasks effectively may help the women perceive the menopause positively. Thus, the quality of life of women may be raised and their hopelessness may be decreased thereafter. It should be kept in mind that the hopelessness is affected by the quality of life. Today where the life span is extended by improving conditions of life, all the medical staff have important roles in enabling women to lead a life of best quality after menopause.
