In this study, we investigated the role of gender on the effect of neostigmin used for reversing the nondepolarizing block. After obtaining the approval of hospital ethics committee and the written patient consents, 100 patients, between 18-70 years, planned for general anesthesia in ASA risc groups I and II were included in the study. Systolic and diastolic arterial blood pressures (STA, DTA), heart rates (KAH), peripheral oxygen saturation (SpO2), adductor muscle temperature and nasopharengeal temperature values were recorded during the procedure. The patients were divided into two groups as female group (n=50) and male group (n=50) and they were premedicated with 0,07 mg/kg of midazolam. TOF nerve stimulator was applied (TOF-WATCH® SX) to all the patients arrived into the operation room. Anesthesia induction was performed with propofol 2 mg/kg and fentanyl 2 mcg/kg. Then, desflurane was started with mask inhalation. Following the loss of eye lash reflex, three consecutive single twitch (Stwitchk) control values were recorded. Rocuronium 0,5 mg/kg was injected after control TOF value (TOFk) was recorded. The duration of TOF value to be zero (TOF0) was recorded. When the TOF ratio recovered to 25 % (TOF25), rocuronium injection was repeated in the dose of ¼ of the initial dose. Repeat amount (TRmg), repeat time (TRt) and repeat number of the rocuronium (TRs), repeat dose TOF25 and TOF ratio at the surgery end (TOFs) were also recorded. After the administration of neostigmine (40 mcg/kg) and atropine (0,02mg/kg), the duration for the TOF ratio reaching to 90% from 25 % (TOF25-90) was also recorded. Whwn the TOF ratio became 90% desflurane was stopped and the duration to extubation(Et) was recorded. Neuromuscular clinic for evaluation purposes; head lifting (BK) and tongue protrution (DÇ) tests were repeated every three minutes for 15 minutes after extubation. There were no differences between the groups according to the demographic data and surgical duration. STA and DTA values of the both groups were found to be decrease in first 25 minutes of the operation. (p<0,05). The KAH values of both groups were observed to be decreased during the operation (p<0,05). TOF0 of the male group was longer than that of the female group (219,34 sec vs. 138,84 sec, p<0.05). In the male group TRt was shorter and TRs was higher (p<0,000). TOF25-90 of the male group was shorter (p=0,02). There was no significant correlation between the TOF25-90 and body mass index (BMI). Mail patients? success was earlier in DÇ and BK tests when compared with the female patients? results (p<0.05). In conclusion; gender may change the effect of neostigmine used for reversal of the nondepolarizing block due to rocuronium and nondepolarizing block recovers earlier in men after the neostigmine administration. Key Words: Nondepolarizing block, reversal effect, gender, neuromuscular monitorization, rocuronium, neostigmine.
