Risk Factors in the Development of Malignant MelanomaObjective: Melanoma is a skin cancer with a high morbidity and mortality rate, with an increasing frequency in the last decades. It is frequently observed in the young population. Therefore, knowing the risk factors that play a role in the development of MM and providing the necessary approach to the patients at risk is important for public health. In our study, it was aimed to identify these risk factors in Turkish society and to determine the findings that may direct the physician and the patient in early diagnosis and treatment.Material and Methods: Our study included 234 patients aged 18 and over who were followed up in Ankara University Faculty of Medicine, Department of Skin and Venereal Diseases and diagnosed with MM between January 1, 2018 and December 31, 2019. At the time of clinical presentation of the cases, sociodemographic and self-reported findings based on history were questioned, and clinical findings were recorded. All body examinations of the cases were performed by hand dermatoscope and recorded by two different researchers. Results: 115 women and 119 men, were included in this study. The mean age at the time of diagnosis was 48.80 and 52.78 for females and males, respectively. 59.8% of the cases were university graduates and 40.2% had high school or lower education levels. The level of education in MIS, YYMM, NMM groups was high and low in LM + LMM and ALM groups (p> 0.001). It was found that 78.2% of the cases lived in the city and 21.8% lived in the countryside and the rate of living in the countryside in the ALM group was significantly higher than other MM subtypes (p <0.001). 67.1% of the cases worked in indoor workers and 15.4% in outdoor workers. Outdoor workers's rates were found to be high in the ALM and LM + LMM group (p> 0.001). There was a history of smoking in 43.5% of the cases and no significant difference was found between the subtypes in terms of smoking rates (p> 0.05). Consuming alcoholic beverages have been reported in 26.9% of the cases and rate in the MIS group was 46% and was significantly higher than other MM subtypes. History of trauma have been recorded in 17.94% of the cases, and the rate of trauma was foundto be significantly higher in NMM (27.6%), ALM (83.3%), others (19%) subtypes (p <0.001). 63.7% of the patients had a history of sunburn for the first time before 13 years of age, 10.3% between the ages of 13-17, and 19.7% of them after 18 years of age. Rates of giving sunburn history for the first time in 13 years and older; MIS (73%) and YYMM (68.3%) were significantly higher in the group and significantly lower in the ALM group (45.8%) (p <0.05). In 61.1% of the cases, there was a history of having sunburn at least once in a period of life, and the average number of burns with bulla was 3.22. While the mean blistered sunburn numbers were significantly higher than the mean in the group of MIS (2.89), YYMM (3.45), NMM (3.57) and others (3.34); it was significantly lower than the mean in the LM + LMM (2.11) and ALM (0.63) groups (p <0.001). Compared to cases with MPM (2.52) and cases with SPM (4.89), it was found that the number of sunburned blisters experienced in patients with MPM was significantly higher (p <0.001), the rate of sunburn seen for the first time at the age of 18 and over SPM was found to be 21.7% in the group and 3.7% in the patients with CPM (p <0.014). 9.4% of cases had a history of immunosuppression and 11.5% had NMSC. The rates of MM occurrence in 1st, 2nd and 3rd degree relatives of the cases were found to be 7.6%, 3.4%, and 2.5%, respectively. BMI of patients in the MIS group was found to be significantly lower than those in other subgroups (p <0.001). The proportions of the cases with fair skin type are in the subtypes, respectively; 61.9% for MIS, 55% for YYMM, 81% for NMM, 51.4% for LM + LMM, 25% for ALM, 51.7% for others, and cases with fair skin type in the NMM group (p <0.001), in the ALM group, the majority of cases had dark skin type (p <0.001). When the cases were evaluated in terms of hair and eye color, there was no statistically significant difference between the whole group and subtypes (p <0.187). The subtypes of MIS, YYMM, NMM, others are most commonly located in the trunk (68.3%, 50%, 33.3%), respectively, LM and LMM (56.8%) in the head and neck region, and ALM was mostly located in lower extremities (83.3%). Locations of the primary tumors didn't differ between genders according to the subtypes (p> 0.05). TBNC of the cases was found as 120.54. While SPM patients was present in 207 (88.5%) of the cases, 27 (11.5%) of them were found to have MPM. TBNC of the patients with SPM and MPM were found as 103.05 and 254.66, respectively. TBNC was found to be significantly high in MPM patients compared to SPM patients (p <0.0001). ALM group had significantly lower TBNC compared to non-acral MM group (p <0.001). The mean number of nevi in the arms in subtypes varied between the numbers of 13.3 and 64.81. The average number of arm nevi was found as 64.80 for MIS, 53.35 for YYMM, 32.42 for NMM, 25.16 for LM + LMM, 13.29 for ALM and 57.62 for others. Total number of nevi in the arms was significantly lower in NMM, LM + LMM, ALM groups compared to other subtypes (p <0.001). The mean number of trunk nevi was found as 70.84 for MIS, 53.32 for YYMM, 29.67 for NMM, 21.05 for LM + LMM, 8.79 for ALM, and 55.00 for others, and it was significantly lower in the LM + LMM and ALM groups (p <0.001 ). The mean number of leg nevi was 26.48 for MIS, 23.48 for YYMM, 17.05 for NMM, 10.08 for LM + LMM, 2.58 for ALM, and 15.10 for others, and the number of leg nevi was significantly lower in the LM + LMM and ALM group (p < 0001). Average head and neck nevi number; 14.67 for MIS, 13.48 for YYMM, 5.57 for NMM, 3.43 for LM + LMM, 4.71 for ALM, 7.86 for others, and head and neck nevus counts were significantly lower in the LM + LMM and ALM group (p <0.001) . TVNS has showed very strong correlation with the total number of nevi in the arms (R2 = 0.920), right arm (R2 = 0.887), left arm (R2 = 0.913), and body (R2 = 0.855) nevus; moderate correlation with the number of leg nevus (R2 = 0.691) and the number of head and neck nevus (R2 = 0.466). SL density was significantly different among different MM subtypes (p> 0.001). Moderate-to-severe SL density rates were as follows according to subtypes; 66.7% for MIS, 71.7% for YYMM, 71.7% for NMM, 75.7% for LM + LMM, 37.5% for ALM, and 65.6% for others. MIS, YYMM, NMM, LM + LMM subtypes had a significantly higher moderate-severe solar lentigo incidence in other groups. ALM group has a significantly low moderate-to-severe SL density (33.3%) within the group (p <0.05).Conclusions: Risk factors of MM development shows variety according to subtypes. The geographic distribution of MM subtypes differs in different populations. For the early diagnosis and treatment of these cases, it is important to define risk factors and apply into clinical settings. There is a need for studies in large cohorts which these risk factors are searched.
