Amaç: Kontrast madde nefropatisi (KMN), akut böbrek hasarlanması açısından riskaltındaki hastalara kontrast madde uygulanması sonrası ortaya çıkan kısmen sıkgörülen, ciddi bir komplikasyondur. KMN gelisen hastalarda hastane içi olay veölüm insidansı yüksek oldugu için bu komplikasyonu önlemek amacıyla pek çokönleyici tedaviler denenmistir. Fakat bugüne kadar KMN riskini azaltmada faydasıkabul edilmis tek yaklasım hidrasyondur. KMN'nin patofizyolojisi kesin olarakolarak anlasılamadıgı halde, vazokonstriksiyon, oksidatif stres, bozulmus nitrik oksidüretimi ve apoptoz KMN'nin patogenezinde suçlanan faktörlerden bazılarıdır.Nebivolol anti-apoptotik ve anti-oksidan özellikleri olan üçüncü kusak bir betablokerdir. Ayrıca nitrik oksid düzeyini arttırarak vazodilatatör etki de gösterir. Busebepten, bu çalısmanın amacı koroner anjiografi sonrası KMN gelisimini önlemedeprofilaktik nebivolol kullanımının etkinliginin N-asetil sistein (NAC) ilekarsılastırılmasıydı.Hastalar ve yöntem: Elektif koroner anjiografi plânlanan, bazal kreatinin düzeyi 1.2ile 1.7 arasında olan 99 hastayı prospektif olarak çalısmaya aldık. Hastalar üç grubaayrıldı. Grup I (NAC grubu) N-asetil sistein verilen ve hidrasyon uygulananhastalardan, grup II (nebivolol grubu) nebivolol verilen ve hidrasyon uygulananhastalardan, grup III (hidrasyon grubu) ise sadece hidrasyon uygulanan hastalardanolustu. Koroner anjiyografiden önce, Grup I'deki (NAC grubu) hastalara,anjiyografiden 2 gün önce baslanarak ve anjiyografi günü sabahı da devam edilerekgünde iki kez olacak sekilde toplam 5 doz (600mg/doz, n=33) NAC verildi. GrupII'deki (nebivolol grubu) hastalara ise yine anjiyografiden iki gün önce baslanarak veanjiyografi günü sabahı da devam edilerek günde bir kez (5mg/doz, n=33) olacaksekilde toplam 3 doz nebivolol verildi. Nebivolol grubunda, islemi takip eden gün birixdoz daha nebivolol verildi. NAC grubunda ise, NAC'a islem sonrası anjiyografininyapıldıgı aksam ve ertesi gün olacak sekilde toplam 3 doz daha devam edildi.1ml/kg/saat'den sodyum klorür infüzyonu islemden 4-6 saat önce baslanarak toplam18 saat olacak sekilde grup III dahil tüm hastalara uygulandı. Bazal kreatinindüzeyine islemden önce bakıldı, ikinci ve besinci gün ölçümler tekrarlandı. KMNtanımlanmasında iki farklı tanımlama kullanıldı. Birinci tanımlamaya göre KMNkreatinin düzeyinde 2. veya 5. gün 0.5 mg/dl veya >%25 artıs olarak belirlendi.kinci tanımlamaya göre ise KMN kreatinin düzeyinde bazal degere göre islemsonrası 2. veya 5. gün 0.5 mg/dl artıs olarak tanımlandı.Bulgular: Tedavi grupları bazal klinik özellikler bakımından birbirine benzerdi.Bazal kreatinin düzeyi grup I'de (NAC grubu) 1.42 ± 0.14, grup II'de (nebivololgrubu) 1.39 ± 0.13 mg/dL, ve grup III'de (hidrasyon grubu) 1.41 ± 0.14 olarakbulundu (p=0.15). kinci ve besinci gün kreatinin düzeyleri arasında üç grup arasındaanlamlı fark yoktu (sırasıyla, p=0.96 and p=0.79). lk tanımlamaya göre KMN, grupI'de %18.2 (6/33), grup II'de %24.2 (8/33) ve grup III'de %24.2 (8/33) olarakbulundu (p=0.79). Bu tanımlamaya göre diyabetik hastalarda da KMN insidansı üçgrup arasında farklı degildi (p=0.16). kinci tanımlamaya göre de KMN insidansı üçgrup arasında farklı olmadıgı halde (p=0.30), diyabetik alt grupta, nebivolol alanhastaların %20 (2/10)'sinde, hidrasyon uygulanan hastaların %55.6 (5/9)'sinde KMNgelisti ve NAC grubundaki hiçbir hastada nefropati gelismedi (p=0.01). Bu istatistikifark NAC grubundan kaynaklanıyordu.Sonuç: Hidrasyon tedavisine ilave olarak kullanılan nebivolol ve NAC elektifkoroner anjiografi uygulanan renal fonksiyonu bozuk hastalarda KMN riskiniazaltmadı. Bu çalısma sadece diyabetik hastalarda, KMN gelisimini önlemedeNAC'ın koruyucu etkisi olabilecegini göstermistir.Anahtar Kelimeler: Kontrast madde nefropatisi, nebivolol, N-asetil sistein.

Aim: Contrast-induced nephropathy (CIN) is a relatively common and potentiallyserious complication occurring after the administration of contrast materials topatients at risk of acute renal injury. Because a higher incidence of in-hospital eventsincluding death has already been described in patients who developed CIN,numerous preventive measures have been evaluated to avoid this undesired outcome.However, to date, the only established approach in the reduction of the risk for CINis volume expansion. Although the pathophysiology of CIN is not exactlyunderstood, vasoconstriction, oxidative stress, impaired nitric oxide production andapopitosis are some of the mainly contributing factors to the pathogenesis of CIN.Nebivolol is a third generation beta blocker which has anti-apoptotic and anti-oxidantproperties. In addition, it has vasodilatator effect via increasing nitric oxide level. So,the aim of the study was to compare the effectiveness of prophylactic nebivolol useversus pretreatment with N-Acetylcysteine (NAC) for prevention of contrast-inducednephropathy after coronary angiography.Patıents and methods: We prospectively enrolled 99 patients who were scheduledfor elective coronary angiography procedure and whose baseline creatinin levelsbetween 1.2 and 1.7 mg/dl. Patients were divided into three groups. Group I (NACgroup) consisted of the patients received N-Acetylcysteine plus hydration, group II(nebivolol group) consisted of the patients received nebivolol plus hydration and,group III (hydration group) consisted of the patients received only hydration. Ingroup I (NAC group), patients received oral NAC (600 mg/dose, n=33) twice a dayfor total 5 doses before coronary angiography, starting two days before angiographyand continuing at the morning of angiography day. In group II (nebivolol group),patients received oral nebivolol (5 mg/dose, n = 33) once a day for total 3 dosesxibefore coronary angiography, starting two days before angiography and continuing atthe morning of angiography day. Nebivolol was also given 1 dose at the dayfollowing the procedure in the nebivolol group. However, in the NAC group, NACwas maintained for total 3 doses after procedure, one dose at the evening ofangiography day and two doses at the next day. 1 ml/kg/h sodium chloride infusionwas administered for 18 hours in all patients including group III (hydration group)(n=33), starting 4-6 hours before the procedure. Baseline creatinine level wasobtained prior to the procedure and repeated 2 to 5 days later. Two definitions wereused for CIN. CIN was defined as an increase of 0.5 mg/dl or an increase of >25%in serum creatinine over baseline at 2-day or 5-day after coronary angiography in thefirst definition. But, CIN was defined as an increase of 0.5 mg/dl in serumcreatinine over baseline at 2-day or 5-day after the procedure according to the seconddefinition.Results: Treatment groups were similar with respect to baseline clinicalcharacteristics. Baseline creatinin was 1.42 ± 0.14 in group I (NAC group), 1.39 ±0.13 mg/dL in group II (nebivolol group), and 1.41 ± 0.14 in group III (hydrationgroup) (p=0.15). Serum creatinine levels at 2-day and 5-day were not significantlydifferent in the three groups (p=0.96 and p=0.79, respectively). According to firstdefinition, CIN occurred in 18.2% (6/33) of patients in group I, 24.2% (8/33) ofpatients in group II, and 24.2% (8/33) of patients in group III (p=0.79). Also, theincidence of CIN in diabetic patients was not different in the three treatment groupsin this definition (p=0.16). Although the incidence of CIN was not different in thethree groups according to second definition (p=0.30), in the diabetic subgroup, CINoccurred in 20% (2/10) of patients taking nebivolol, none of patients taking NAC and55.6% (5/9) of patients administered only hydration (p=0.01). This statisticallysignificant difference was due to NAC group.Conclusion: Both nebivolol and NAC which was added to hydration theraphy didn?treduced the risk of CIN in patients with reduced renal function undergoing electivecoronary angiography. This study demonstrated the protective effect of NAC on CINin only diabetic patients.Key Words: Contrast-induced nephropathy, nebivolol, N-acetylcysteine.
