7. SUMMARY Effects of Mandibular Midline Distraction Osteogenesis on the Dentofacial Structures Transverse skeletal deficiencies are common clinical problems usually associated with narrow basal bone and dentoalveolar structures, and dental crowding. Transverse mandibuler deficiencies in the mix dentition arc commonly corrected with removable orthodontic expansion devices, lip bumpers or functional devices. However in the permenat dentition, mandibuler dental arch expansion will be unstable and discrepancy will tend to relapse towards its original position. The traditional approaches for correcting mandibular crowding are extraction of teeth, interproximal reduction of tooth mass and dental compensations therapies. Distraction osteogenesis is a biological process of new bone formation between bone segments that are gradually separated by incremantal traction. The traction generates tension that stimulates new bone formation paralel to the vector of distraction. This procedure has become a popular alternative technique to treat craniofacial skeletal dysplasias in 1990's, and Guerrero pioneered the use of rapid mandibular expansion to correct mandibular transverse discrepancies in 1990. The purpose of this study is to evaluate the effects of mandibular midline distraction osteogenesis procedure on the dental and skeletal structures and mandibular condyle, by using a tooth and bone borne distraction device. 96The sample comprised of 20 patients, aged between 15.3 and 24.9 years with a mean age of 20. 1 year at the start of treatment. All patients required mandibular midline distraction osteogenesis, based primarily on moderate to severe mandibular anterior crowding, V shaped mandibular arch form and maxillo-mandibular transverse deficiency. With the use of metallic implant technique described by Björk (1968), 12 subjects had 6 metallic implants placed in the mandible during the surgical operation. A custom made intraoral, bone and tooth-borne distraction device was used for distraction. After the latency period of 7 days, the distraction device was activated by the patient twice a day once in the morning and once in the evening, with a total amount of 1 mm per day. The device was usually maintained in position about 90 days after surgery for maturation of new developed bone (consolidation period). After the completion of distraction period, the lower anterior teeth were bonded and tooth movement to the distraction site was initiated. At the end of the consolidation period, lower arch crowding was solved and the teeth were aligned. Rapid palatal expansion procedure was also completed in the consolidation period, in all of the study cases. The records were obtained at the start of treatment, at the end of distraction (10.75±2.21 days after surgery) and consolidation periods (94.95±7.47 days after surgery). These records were included postero-anterior, submentovertex and lateral ccphalograms and study casts. The data was analyzed statistically by using the repeated measure analysis of variance and paired t-test. Friedman chi-square and Wilcoxon signed ranks tests were also used to assess the implant measurements. 97Postero-anterior cephalometric analysis demonstrated that no significant skeletal changes were observed in the bigonion, biantigonion widths and the interramal implant distance as well as the intcrramal and implant angles, at the end of distraction period. On the other hand, the intersympseal and intermolar implant distances and the bimolar width were significantly increased while bicondylar width was markedly decreased. Post distraction submentovertex cephalometric analysis demonstrated that the intercondylar angle was increased and the intercondylar distance was decreased significantly. The dental cast model analysis indicated widening effect gradually decreased from the mandibular canines to the second molars. Lateral cephalometric evaluation showed significant increases in the vertical measurements and proclination of the lower incisors was demonstrated. Dental crowding was resolved by the movement of the teeth into the distraction gap with fixed appliance orthodontic treatment. In conclusion, the short term results of this study would indicate that mandibular midline distraction osteogenesis procedure provides an efficient non- exraction treatment alternative for mandibular dental crowding, by increasing the mandibular skeletal and dental arch witdhs. 98.
